Uniform Coverage Documents
Consumers have access to two forms to help them understand and evaluate their health insurance choices, including:

  • An easy-to-understand Summary of Benefits and Coverage with coverage examples that illustrate what a plan would pay in two different examples
  • A uniform glossary of terms commonly used in health insurance coverage, such as "deductible" and "copay"

W-2 Reporting
Employers must report the value of health coverage they provide on employees' W-2 forms. These amounts are not taxable. Learn more in this FAQ.

The Patient-Centered Outcomes Research Fee
Health insurers and self-funded plans pay an annual fee to fund research on improving healthcare delivery and outcomes. The annual fee for each person covered by the plan is:

  • $1 for plan years that end between October 1, 2012 and September 30, 2013
  • $2 for plan years that end between October 1, 2013 and September 30, 2014

The fee will be adjusted for annual increases until it ends in 2019.
