Four coverage levels to choose from

There are four coverage levels available to meet your needs: $5,000, $10,000, $15,000, and $20,000. At the $5,000 coverage level, you are guaranteed insurance. The plan covers the most common critical illnesses:

  • 100% of policy limit: Heart attack, stroke, invasive cancer, end-stage renal (kidney) failure, major organ transplant
  • 25% of policy limit: Pre-cancerous carcinoma in situ (CIS), prostate cancer
  • 5% of policy limit: Skin cancer

All coverage levels offer benefits like affordable premiums, no deductible, guaranteed renewal through age 69, a re-occurrence benefit, and dependent coverage. Optional benefits are available as well.

This plan is a specified disease policy. It provides limited benefits, and contains exclusions and limitations, including pre-existing condition limitations and waiting periods. This plan will not pay benefits for a diagnosis of a critical illness that is caused by a pre-existing condition, unless the date of diagnosis is more than 12 months after the effective date of coverage. You will receive only the benefit level specified in the policy you selected and was issued to you, and your premium will vary according to the benefit level you selected.
