Employer Size Certification Process New Business Submissions

To receive a certified quote, the group will need to provide:

  1. Their most recent payroll or quarterly tax & wage report
  2. Waivers for any eligible employee or dependent(s) of eligible employee not currently enrolled on the group plan
  3. Enrollment application for each employee enrolling onto the plan

Any discrepancies in the above documentation will be brought to the attention of the submitter for clarification prior to the release of a rate.

Small groups must meet the participation requirements to receive a small group quote: 75% for groups of 1-50 eligible employees.

Upon the sale of the group, the General Agents and Intermediaries, as applicable, will submit the signed sales agreement with the check, membership applications, and a broker of record letter to the Small Group Underwriting Department for verification of actual enrollment. BrokerNet representatives will perform quality control to ensure accurate enrollment. Any group that is enrolling a population that is different from that originally submitted, but still meets minimum participation requirements will receive a revised quote and will be required to submit waivers for any employee originally submitted but choosing not to enroll. Any group whose actual enrollment does not meet minimum participation requirements will not be accepted.
