How to Quote New Business
The required materials must be submitted to BCBSRI via your General Agent for groups of 1 to 50 eligible employees. All other groups can be submitted directly to BCBSRI. The deadline for submissions for groups of 1 to 50 is the 23rd of the month preceding the effective date.
A. Small Group (1-50 "eligible employees")
- Most recent payroll or quarterly tax & wage report
- Waiver of Coverage forms for all eligible employees not currently enrolled on the group plan
- Enrollment Applications
- Broker of Record Letter
B. Large Group (more than 51 "eligible employees")
Group size 51 – 199 eligible employees:
- Employee Census showing full names, gender, DOBs, and status (Individual, Employee Plus Spouse, Employee Plus Children, Family) for ALL eligible employees
- Group Risk Appraisal Form (previous 12 months claims experience if available)
- Employer contribution
- Workers' compensation carrier
- Current carrier bill
- Present (and previous) benefit design, rates, renewal date
- Certification of employer size if total covered is less than 51 but total eligible is more than 51
- Contact Blue Cross representative for additional information
Group Size 200+ eligible employees:
- Employee Census showing full names, gender, DOBs, and status (Individual, Employee Plus Spouse, Employee Plus Children, Family) for ALL eligible employees and individual or family status
- Previous 12 months claims experience and enrollment
- Current carrier bill
- Workers' compensation carrier
- Large claims report
- Present benefit design, rates, renewal date
- Employer contribution
- Contact Blue Cross representative for additional information
Note to Broker: The materials requested include information protected as confidential under Rhode Island and Federal law. Safeguard its confidentiality. Limit access to the individuals necessary for processing.
How to quote new dental business*
(Blue Cross Dental must be the sole dental benefit carrier)
Group Size 3 - 50 (enrolled employees):
- Eligible employee count, enrolled employee count, spousal waiver count (enrollee only, enrollee and spouse, enrollee and child(ren), or enrollee, spouse and child(ren), contracts, present dental carrier, length of coverage, benefit design in force, SIC code or type of industry, plan being quoted, and rate period to be quoted.
Group Size 51 + (enrolled employees):
- Eligible employee count, current Individual/Family contract codes, spousal waiver count, present dental carrier, billing statement, length of coverage, benefit design in force, plan SIC code or type of industry being quoted, present and renewal rates, employer contribution, and rate period to be quoted.
- Previous 12 months experience, if available, including claims, enrollment, rates, and any supplemental competitor exhibits.
* The Essential Dental Plan cannot be quoted as a stand-alone product. It must be quoted as a rider to a health product.
