1. Participation Requirements (enrolled employees):

A minimum of three (3) enrollees is required for dental coverage.These participation requirements must be met, with the exception of legitimate waivers. In these circumstances, waiver cards must be completed, however, a minimum of three (3) enrollees is still required for dental coverage.
