We’re committed to helping our members be as healthy as possible, and to providing resources to help them better manage their health. That’s why our Medicare Advantage plans cover a comprehensive in-home health assessment at no cost!

Why have an in-home assessment?

By participating in this no-cost benefit, your loved one will:

  • Have a dedicated one-on-one session with a qualified medical professional (a doctor or nurse practitioner) about their unique health needs.
     
  • Receive a thorough review of all of their medications, including guidance to make sure they’re taking them correctly and safely.
     
  • Be better prepared for their next visit with their doctor (who will also receive a health summary after the assessment) to provide a better picture of their overall health and healthcare needs.
     
  • Have the option to participate in our programs that can help them manage their care and any chronic conditions they may have.
