We offer programs that can show you how to manage your conditions and feel more in control of your health. Their clinicians will work with you and your providers to get you on the path to a healthier life.

Diabetes
A personalized online program, through Livongo by Teladoc Health®, includes diabetes testing supplies, condition management, and expert coaching. We can help you with:

Congestive heart failure
Receive easy-to-use monitoring devices through our partnership with Advanced Monitoring Caregiving (AMC) Health. With these free devices, your nurse can remotely keep track of your congestive heart failure and intervene quickly. These devices can be helpful in reducing emergency room visits and possible hospital stays. If you qualify, you’ll get:
- Special scale to track your weight
- Blood pressure cuff to track your blood pressure
- Smart tablet for education and assessments
- Nurse assigned specifically to you who can coach and support you
COPD
You can get coaching and support to help you cope with chronic obstructive pulmonary disorder through our partnership with Advanced Monitoring Caregiving (AMC) Health. With easy-to-use monitoring devices, your nurse can remotely keep track of your condition and intervene quickly if needed. Qualifying BCBSRI members can receive:
- Special inhaler sensor to track medication use
- Pulse oximeter to track oxygen saturation
- Smart tablet for education and assessments
- Nurse assigned specifically to you who can coach and support you

Health assessments
BCBSRI offers free health assessments in your own home or at your nearest Your Blue StoreSM, wherever you feel more comfortable. These screenings, conducted by Care Connectors Medical Group, are:
- Helpful – Get valuable information to help you and your provider make decisions.
- Convenient – Receive your screening at home or any nearby Your Blue Store.
- Personal – Enjoy 30-45 minutes of one-on-one time with a licensed clinician.
