We work with your healthcare team to help you:

- Understand the diagnosis
- Review your care options, including quality treatment close to home
- Manage your symptoms
- Find mental health and social services if needed

Daymark Health, Inc. ("Daymark") is an independent company contracted by Blue Cross & Blue Shield of Rhode Island (BCBSRI) to provide care coordination services. Daymark has contracted with Daymark Health DE LLC ("Medical Group") to enable access to Medical Group's employed licensed providers to provide clinical services.
