BCBSRI is committed to developing innovative programs that collaborate with you to support patients. These programs can help them navigate the challenges of complex or advanced illness, support chronic conditions outcomes, and reduce hospitalizations. These programs also can help you:

  • Enhance care coordination, working with your team to help members get the right care at the right time
  • Improve patient outcomes through enhanced support and wrap-around services
  • Reduce administrative burden by helping with the complexities of navigating care

All with no additional cost to providers or patients for program participation.

Conditions and programs

Cancer support

Program: Daymark Health

  • Navigation and support for cancer patients
  • 24/7 virtual and in-home care, symptom management, mental health, and social support
  • Complements provider care for seamless experience

Kidney disease (CKD 4-5, ESKD)

Program: Interwell 360

  • Team-based care for advanced kidney disease
  • Clinicians, social workers, and advocates collaborate with providers
  • Individualized care plans, in-person or phone engagement

In-home complex care

Program: HouseCall by Blue (ConcertoCare)

  • In-home and virtual care for patients with multiple medical needs
  • 24/7 support for patients, families, and caregivers
  • Teams can include physicians, nurse practitioners, behavioral health providers, pharmacists, care managers, and social workers
  • Providers receive regular, real-time updates and reports

Preventive assessment & screening

Program: Care Connectors

  • Health assessments and preventive screenings in homes, mobile clinics, assisted living, telemedicine, and embedded PCP offices
  • Nurse practitioners review history, recommend screenings, and perform point-of-care testing
  • Monthly visit summaries for providers

Congestive heart failure (CHF) & COPD

Program: Advanced Monitoring Caregiving (AMC)

  • Remote patient monitoring for CHF and chronic obstructive pulmonary disease (COPD)
  • Patients receive easy-to-use devices (scale, blood pressure cuff, inhaler sensor, pulse oximeter, smart tablet) for home use
  • Assigned nurse provides coaching, monitors readings, and shares data with providers
  • May help reduce emergency visits and hospitalizations
  • Providers can refer eligible patients directly

Behavioral health

Program: BCBSRI Behavioral Health Case Management program

  • Staffed by experienced mental health and substance use professionals
  • Education and support
  • Coordination of care
  • Help to close gaps related to social determinants of health

Diabetes management

Program: Livongo by Teladoc Health

  • Patients receive a connected glucose meter and unlimited strips/lancets
  • Readings upload to a secure account for provider access
  • Health coaches are available for nutrition and lifestyle support

ConcertoCare is an independent company contracted by Blue Cross & Blue Shield of Rhode Island (BCBSRI) to provide in-home healthcare support services.

Care Connectors Medical Group is an independent company contracted by Blue Cross & Blue Shield of Rhode Island (BCBSRI) to provide health assessment services.

Advanced Monitoring Caregiving, Inc. is an independent company contracted by Blue Cross & Blue Shield of Rhode Island (BCBSRI) to provide remote patient monitoring services.
