Effective October 1, 2019, Hyperbariac Oxygen Therapy will no longer require prior authorization for both BlueCHiP for Medicare and commercial products. Services are medically necessary when filed with a covered diagnosis. To ensure correct claims processing, the applicable diagnosis must be filed in the claim line.
