This policy was updated on April 20, 2022 to include new HCPCS codes effective April 1, 2022.

Effective July 1, 2022, CPT codes 37252 and 37253 will be separately reimbursed for commercial products and Medicare Advantage plans when filed by professional providers with place of service codes 21 (Inpatient), 22 (Outpatient), and 23 (Emergency Department) only. These codes will remain not separately reimbursed when filed in all other instances (e.g., place of service 11). Additional information pertaining to this payment policy can be found here.
