Acupuncture Services for Medicare Advantage Plans

This policy was updated to reflect the new 2026 benefit changes for Individual Medicare plans. Acupuncture is no longer covered effective January 1, 2026, and only Chronic Lower Back Pain is. For additional details related to this policy, please click here.     

Non-Reimbursable Health Service Code Policy

CPT Code 89258 was originally on the use alternate code list and since has been removed for both professional and facility claims effective January 1, 2025. (for both Commercial and Medicare)
