Effective February 1, 2020, CPT code 62380 for Endoscopic decompression of spinal cord, nerve root(s), including laminotomy, partial facetectomy, foraminotomy, discectomy and/or excision of herniated intervertebral disc, (1 interspace, lumbar) will require prior authorization for BlueCHiP for Medicare and commercial products. The service and the corresponding CPT code will be found in the Preauthorization via Web-Based Tool for Procedures policy.
