According to California Blue Cross, plans for Charter Care/Astrada Health were terminated effective 12/31/2025 with California Blue Cross. 

Claims for dates of service (DOS) from 1/1/2026 through 3/31/2026 should be submitted to Personify. Please note that Personify is not affiliated with Blue Cross, and we do not have additional details regarding their claim submission procedures or member ID numbers. The patients should have a new card with their Personify ID and information on how to submit the claims directly to them on the back of their card. 

Charter Care / Astrada Health switched to Blue Cross & Blue Shield of Rhode Island (BCBSRI) effective 4/1/2026. For claims with DOS on or after 4/1/2026, these claims should be filed using the member’s new BCBSRI ID and submitted directly to BCBSRI.
