Some fields in MHK will be moved effective May 3, 2021:

• The “Contact Name” field will be moved to a new line under the “ZIP Code” line.
• An additional “Contact Phone” field for the contact person has been added. It is not mandatory to submit the authorization, but is an enhancement to capture the exact phone number of the contact if it is different from the provider’s office number.
