Online

You can enroll online. You’ll just need a few documents.

  • Medicare card – the blue-and-white card with your Medicare number
  • Certificate of Credible Coverage – for Part D prescription drug coverage; you will need to submit separately
  • Notice of Replacement of Medicare Supplement Form (NOR) – you might need to submit this separately

When you are done, you will see further instructions for submitting documents separately.
