HealthMate for Medicare (PPO) members: Next steps

HealthMate for Medicare (PPO) will be discontinued after December 31, 2024, so you will need to choose a new plan for 2025.

This page explains options that could be a good fit for you and your health needs. The brochure you received in the mail includes charts that give more detailed comparisons.

NOTE: If you don’t take action before December 31, 2024, you will lose your prescription drug coverage and only be covered by Original Medicare (Parts A and B—hospital and medical) starting January 1, 2025. Medicare requires you to have "creditable coverage" for Part D drugs.

Consider these options to replace your HealthMate for Medicare plan

BlueCHiP for Medicare

Extra (HMO-POS)

Extra is a traditional Medicare Advantage plan. Like HealthMate, it includes coverage for medical, hospital, drugs, and dental (including crowns), as well as vision, hearing, and other supplemental benefits.

Consider Extra if...

  • You prefer all of your coverage and benefits in one plan, with one monthly premium.
  • You receive much your care in-network, but you enjoy the flexibility of an out-of-network benefit. See if your doctors are in the Extra network.
  • You use your dental, vision, and hearing benefits through your current plan, as well as extras like the Flexible Benefit Card and gym membership.

Medicare Supplement

Plan 65

Plan 65 is a Medicare Supplement plan, also known as Medigap. It covers most of the deductibles and copays that Medicare Parts A and B don’t, limiting your out-of-pocket expenses while giving you the choice of any provider or hospital that accepts Medicare—without needing a referral.

Consider Plan 65 if...

  • You prefer lower out-of-pocket costs in exchange for a higher up-front premium.
  • You spend much of your time and/or seek most of your care out of state.
  • You are willing to purchase additional plans for drug and dental coverage, as well as vision.
