A patient-centered medical home (PCMH) is a team led by your doctor that's committed to improving your health and helping you live a healthier lifestyle.
In addition to your doctor, your team can include nurses, specialty physicians, pharmacists, a nutritionist, care coordinators and others depending on your needs.
Your PCMH can help you achieve your health goals. For example, they might help you quit smoking, control your diabetes or asthma, lose weight or manage your stress.
You have access to a team of healthcare professionals. The team in your PCMH looks at your whole health. You have expanded access to care, which may include evenings and weekends. Your doctor's office follows up with you after hospitalizations and assists you in scheduling visits or tests. If you need a specialist for a specific condition, your PCMH will help you find one and communicate with the specialist directly.

Start by making an appointment with your doctor. Your doctor's office will set up any care needed from other team members.
Yes. You're not required to receive care at a PCMH. However, visiting your PCMH as your primary place of care helps your doctor better manage your care with your specialists.
